Job Summary
Conduct home health visits to administer services, perform comprehensive assessments, and update physician orders for patients in Bakersfield, Lancaster, and surrounding So-Cal areas. Manage patient treatment plans, coordinate care with the Clinical Team Manager and physicians, and supervise Home Health Aides per regulatory requirements. Document all activities, upload entries to the Synergy Software portal, and handle re-certifications, transfers, and discharges as requested. Work at least two to three days weekly with flexibility in scheduling, including one weekend and two holidays annually. Earn $50.00 to $140.00 per visit as a W-2 per diem role. Requires a current CA RN license, one year of nursing experience, BLS certification, IV/phlebotomy credentials, and NSO insurance.
Required Qualifications
- 1 year RN experience
- 1 year nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
