Job Summary
Conduct home health visits to administer IVs, change dressings, clean wounds, and perform comprehensive patient assessments. Document all findings and upload records to the Synergy Software portal while obtaining and verifying physician orders. Initiate, re-evaluate, and revise care plans in consultation with the Clinical Team Manager, coordinating significant changes with physicians and other team members. Manage re-certifications, transfers, and discharges as requested, and supervise Home Health Aides per regulatory requirements. Maintain knowledge of state regulations and submit all related documentation. Requires current CA RN license, BLS certification, and IV/Phlebotomy credentials. Pay ranges from $50 to $140 per visit with a schedule of two to three days weekly plus one weekend and two holidays annually.

- Current CA RN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)

- Obtaining and documenting physician orders
- initiate and regularly re-evaluate and revise the plan of care
- Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team
- Performing re-certifications, resumption of care, transfers, and discharges
- Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA)
- Performing Home Health Aide supervisory visits per regulatory requirements
- Demonstrate knowledge of regulatory requirements
