Registered Nurse (RN) Visiting Home Health

Job Summary

Conduct home health visits to administer services, perform comprehensive assessments, and maintain patient independence. Administer IVs, change dressings, clean wounds, and oversee treatment plans while updating physician orders and the Synergy Software portal. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise care plans, coordinating significant changes with physicians and other team members. Manage re-certifications, resumption of care, transfers, and discharges, including supervisory visits for Home Health Aides. Demonstrate knowledge of regulatory requirements and OASIS documentation. Work seven days a week with flexible scheduling of at least two-three days weekly, one weekend monthly, and two holidays annually.

  • Ability to work at least two-three days each week
  • Ability to work one weekend a month
  • Ability to work two holidays per year
  • Visiting RN for Home Health experience for 1 year
  • 1 year of Nursing experience
  • Current CA RN License
  • BLS from the American Heart Association and/or the American Red Cross
  • NSO Nursing License Insurance
  • IV/Phlebotomy Certified
  • Knowledge of regulatory requirements
  • Ability to perform an initial, comprehensive assessment
  • Ability to document and upload to Agency Portal
  • Ability to obtain and document physician orders
  • Ability to initiate and regularly re-evaluate and revise the plan of care
  • Ability to coordinate care with the assigned Clinical Team Manager, physician and other members of the home care team
  • Ability to perform re-certifications, resumption of care, transfers, and discharges
  • Ability to assess the need for the services of other team members (PT, OT, ST, MSW, and HHA)
  • Ability to perform Home Health Aide supervisory visits per regulatory requirements
