Registered Nurse (RN) Visiting Home Health

Conduct home health visits to administer services such as IVs, wound care, and dressings while performing comprehensive patient assessments. Document all findings and upload records to the Synergy Software portal, obtaining and verifying physician orders. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans, coordinating significant changes with physicians and other home care team members. Manage re-certifications, transfers, and discharges as directed, and supervise Home Health Aides per regulatory requirements. Maintain current CA RN licensure, BLS certification, and IV/Phlebotomy credentials. Work a flexible schedule with availability for weekends and holidays, earning $50–$140 per visit.
