Registered Nurse (RN) Visiting Home Health

Conduct home health visits to administer services such as IVs, wound care, and dressing changes while performing comprehensive patient assessments. Document all findings, update physician orders, and upload data to the Synergy Software portal. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise care plans, coordinating with physicians and other team members regarding significant patient changes. Manage re-certifications, transfers, and discharges as directed, and supervise Home Health Aide visits per regulatory requirements. Maintain current CA RN licensure, BLS certification, and IV/Phlebotomy credentials. Work flexible schedules including weekends and holidays, earning $50–$140 per visit.
