Job Summary
Conduct home health visits to administer services such as IVs, wound care, and assessments while maintaining patient independence. Perform comprehensive initial and ongoing assessments, document findings, and upload records to the Synergy Software portal. Obtain physician orders, revise care plans in consultation with the Clinical Team Manager, and coordinate with the home care team regarding patient changes. Execute re-certifications, transfers, and discharges as directed, while supervising Home Health Aides per regulatory requirements. Work at least two to three days weekly with flexibility for weekends and holidays, earning $50–$140 per visit.
Required Qualifications
- 1 year (Required)
- Nursing: 1 year (Required)
- Current CA RN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)
