Job Summary
Conduct home health visits to administer services, perform comprehensive assessments, and update physician orders for patients. Document all interactions and upload data to the Synergy Software portal while coordinating care with the Clinical Team Manager and other team members. Initiate, re-evaluate, and revise patient treatment plans, manage re-certifications, and supervise Home Health Aide visits per regulatory requirements. Maintain knowledge of state regulations and communicate significant patient condition changes to the assigned clinical team.
Required Qualifications
- 1 year RN experience
- 1 year nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
