Job Summary
Conduct home health visits to administer IVs, change dressings, clean wounds, and perform comprehensive patient assessments. Document all findings and upload data to the Synergy Software portal while obtaining and verifying physician orders. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans, coordinating significant condition changes with physicians and other team members. Supervise Home Health Aides per regulatory requirements and manage re-certifications, transfers, and discharges. Maintain knowledge of state regulations and OASIS documentation standards. Requires current CA RN license, BLS certification, and IV/Phlebotomy credentials. Flexible scheduling includes weekend and holiday work.
Required Qualifications

- Nursing documentation/charting skills
- Fast learner

- Ability to work at least two-three days each week
- Ability to work one weekend a month
- Ability to work two holidays per year
- Visiting RN for Home Health experience
- 1 year RN experience
- 1 year Nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
