Job Summary
Conduct home health nursing visits to administer IVs, change dressings, and perform wound care while maintaining patient independence. Perform comprehensive patient assessments, obtain physician orders, and update care plans in consultation with the Clinical Team Manager. Document all interactions, upload data to the Synergy Software portal, and coordinate with the home care team regarding significant condition changes. Supervise Home Health Aides per regulatory requirements and manage patient transfers, resumption of care, and discharges as requested. Requires current CA RN license, BLS certification, and IV/Phlebotomy credentials.
Required Qualifications
- Clinically experienced nurse
- Nursing documentation/charting skills
- Fast learner
- Great communication skills
- Highly motivated
- Positive-minded
- Ability to work at least two-three days each week (or ten productivity units)
- Ability to work one weekend a month
- Ability to work two holidays per year (one winter and one summer)
- 1 year RN experience
- 1 year Nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
