Job Summary
Provide home health nursing visits to administer services, perform comprehensive assessments, and document findings in the Synergy Software portal. Obtain physician orders, revise care plans with the Clinical Team Manager, and coordinate updates regarding significant patient changes. Execute re-certifications, transfers, and discharges while supervising Home Health Aides per regulatory requirements. Assess needs for additional team members including PT, OT, ST, MSW, and HHA. Maintain current CA RN license, BLS certification, and IV/Phlebotomy credentials. Work flexible schedules including weekends and holidays with a minimum of two-three days weekly.
Required Qualifications

- Nursing documentation/charting skills
- Fast learner

- Visiting RN for Home Health: 1 year
- Nursing: 1 year
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
