Job Summary
Conduct home health visits to administer services, perform comprehensive assessments, and update physician orders for patients in Bakersfield. Manage treatment plans, coordinate with the Clinical Team Manager and care team, and supervise Home Health Aides per regulatory requirements. Document all activities, upload data to the Synergy Software portal, and handle re-certifications, transfers, and discharges. Requires a current CA RN license, BLS certification, IV/Phlebotomy skills, and at least one year of home health experience. Work at least two to three days weekly with one weekend and two holidays annually.

- 1 year of Nursing experience
- Current CA RN License

- NSO Nursing License Insurance
- IV/Phlebotomy Certified
