Job Summary
Provide skilled professional nursing care as a Home Health RN Case Manager to a primary caseload, delivering person-centered care to patients, families, and caregivers in the home setting in accordance with physician orders and Medicare Home Health Conditions of Participation. Requires a Bachelor’s degree in nursing (preferred) and an active Ohio RN license with at least 1 year of direct patient care in home care. Full-time position with no scheduled weekends (PRN).
