Registered Nurse (RN) - Triage Specialist - Conduit Health Partners

RN Triage Specialist provides telephonic triage to determine the most appropriate level of care using workflows and protocols, delivering education and care advice with clinical judgement. Responsibilities include care coordination to reduce readmissions and manage chronic disease, scheduling and facilitating provider communication, and accurate EMR documentation. Must maintain HIPAA-compliant home workspace with secure physical areas, meet IT requirements (high-speed internet), and be prepared for 4-week orientation, a six-week rotating schedule with on-call and every-other-weekend commitments. Requires an active Compact RN license and multistate licensure; ADN or Diploma in Nursing is required with BSN preferred; 1 year of acute care nursing experience with triage experience preferred. Proficiency in telephonic/virtual monitoring, attention to detail, critical thinking, communication, teamwork, and relationship-building is required.
