Job Summary
Registered Nurse (RN) in Transitional Care Coordination providing assessment, planning, implementation, evaluation, and supervision of patient care on a nursing unit. Responsibilities include assessing physical and psychosocial condition, developing a care plan with patient/family and healthcare team, coordinating care and education post-discharge, acting as a preceptor to new staff and serving as charge nurse, with competency in neuro assessment, airway management including tracheostomy suctioning, and care of patients on ventilators. Works under Nurse Manager/Clinical Coordinator and adheres to unit policies and procedures; may transfer to other units as needed. Cardiac ICU/CTICU and vascular/stepdown units described with emphasis on complex cardiac care and readiness for discharge, ongoing education, and commitment to the organization’s values and performance standards.
Required Qualifications
- Graduation from an accredited school of nursing
- Licensed and currently registered nurse in the State of Connecticut
- BSN preferred or higher
- Current BCLS certification or obtain within one month of hire
