Registered Nurse RN – Transitional Care Coordinator

Registered Nurse on a nursing unit under supervision of the Nurse Manager/Clinical Coordinator. Assesses, plans, implements, evaluates and supervises patient care; may function as Charge Nurse and Preceptor; provides supervision of non-RN staff; demonstrates competency in neuro care including neuro assessment, airway management, tracheostomy suctioning, and care of patients with complex cardiac and neuro conditions; collaborates with the healthcare team to develop and document the Plan of Care, facilitates education for patients and families, and supports discharge planning. Roles include ensuring adherence to unit policies, maintaining high levels of service, and contributing to specialty programs such as the TIA teaching program; opportunities exist within Cardiac ICU/Care units with various vascular and postoperative patient populations.
