Registered Nurse RN – Transitional Care Coordinator

RN (Registered Nurse) in Transitional Care Coordinator role providing patient-centered inpatient care across cardiac units. Responsibilities include adherence to ANA Standards of Practice, assessment, diagnosis, outcomes identification, planning, implementation, coordination of care, health teaching and health promotion, and evaluation. Collaboration with patient/family and healthcare team, ethical practice, leadership and professional behavior, and pursuing evidence-based practice and professional development. Requires licensure in Connecticut and BCLS certification, with BSN preferred; supports a high-quality, culturally congruent practice environment in inpatient cardiac care units.
