Job Summary
RN provides professional nursing care within the Connecticut Nurse Practice Act and ANA standards, and serves as a Transitional Care Coordinator. Responsibilities include establishing therapeutic relationships with patients and families, communicating and collaborating with the patient, family and healthcare team, incorporating cultural considerations, and evaluating care using the nursing process (assessment, diagnosis, outcomes identification, planning, implementation, and evaluation). The role emphasizes leadership, delegation, ethics, patient-centered care, advocacy, and coordination of care across transitions, with emphasis on nursing documentation, health teaching, and quality improvement.
Required Qualifications
- Licensed and currently registered nurse in the State of Connecticut
- BCLS certification (current or within one month of hire)
- Graduation from an accredited school of nursing
- BSN preferred
