Job Summary
Registered Nurse – Transitional Care Coordinator provides patient-centered nursing care across the continuum, assessing physical and psychosocial needs, developing and documenting individualized plans of care, coordinating with the healthcare team, and evaluating patient outcomes including readiness for discharge. Responsibilities include supervising non-RN staff, acting as charge nurse, and delivering education to patients and families. The role requires proficiency in neuro assessment, airway management, care for tracheostomy patients, ventilator care, and familiarity with cardiac ICU pathways and teaching programs such as the hospital’s Tia program.
Required Qualifications
- Graduation from an accredited school of nursing
- Licensed and currently registered nurse in the State of Connecticut
- BSN preferred
- BCLS certification required or obtainable within one month of hire
