Registered Nurse RN – Transitional Care Coordinator

Job Summary
RN responsibilities include assessing, planning, implementing, and evaluating individualized patient care; supervision of non-RN personnel; serving as charge nurse and preceptor; coordinating care across neuro and cardiac ICU units; developing and communicating care plans; educating patients and families; and participating in education, professional development, and adherence to nursing standards. Skills highlighted include neuro assessment, airway management, suctioning of pharyngeal/tracheostomy airways, care of trached patients, experience with devices (e.g., halo brace, LVAD/ECMO considerations), and collaboration with physicians and nursing team for post-operative and critical care. The role is based in Cardiac ICU/CCTICU/stepdown units within YNHHS Heart & Vascular services, located in Middletown, OH.
Required Qualifications
- Graduation from an accredited school of nursing
- Licensed and currently registered nurse in the State of Connecticut
- BSN preferred
- Current BCLS certification or obtain within one month of hire
- Nursing practice overseen by CNO in each delivery network
