Registered Nurse RN – Transitional Care Coordinator

Registered Nurse (RN) for Transitional Care Coordinator in a Cardiac ICU/CTICU/Hybrid unit. Responsibilities include assessing the patient’s physical and psychosocial condition, developing and documenting individualized plans of care, coordinating care with the healthcare team, documenting outcomes, educating patients and families, serving as a preceptor to new staff, acting as charge nurse, and ensuring readiness for discharge. Demonstrates competency in neuro patient care, neuro assessment, airway management of neuro patients, and procedures such as suctioning for pharyngeal/tracheostomy care; experienced with devices like LVADs and ECMO. BSN preferred; current state RN licensure and adherence to Nurse Practice Act are expected.
