Job Summary
Registered Nurse (RN) – Transitional Care Coordinator at Yale New Haven Health System. Manages professional nursing care within the patient-centered framework, collaborating with patients, families and health-care teams to ensure quality outcomes. Responsibilities include applying ANA Scope and Standards of Practice, coordinating care transitions, health teaching, and promoting holistic data collection and planning. Required Qualifications
- Graduation from an accredited school of nursing
- BSN preferred
- Licensed (currently) as a registered nurse in the State of Connecticut
- BCLS certification or ability to obtain within one month of hire
- Clinical specialty certification highly encouraged
