Job Summary
Registered Nurse (RN) in Transitional Care Coordination responsible for assessing patients, planning and coordinating care across the continuum, guiding families, supervising non-RN staff, and acting as a preceptor/charge nurse. Requires caring for neuro-focused patient populations, performing neuro assessments, airway management, suctioning, and caring for post-operative cardiac and ICU patients; requires adherence to nursing standards and professional behavior within the CT practice environment. Requires active CT nursing license and BSN preferred; BCLS certification required within one month of hire; complex cardiac/ICU skills and collaboration with multidisciplinary teams are essential.
Required Qualifications
- Graduation from an accredited school of nursing
- BSN preferred
- current BCLS certification or obtain within one month of hire
- Licensed and currently registered nurse in the State of Connecticut
- Clinical specialty certification highly encouraged
