Job Summary
Registered Nurse RN - Transitional Care Coordinator provides ongoing patient assessment, care planning, and coordination across the continuum of care; supervises non-RN personnel, may assume charge nurse responsibilities, and delivers education to patients and families; role includes neuro and airway management, trached patient care, ventilator management, and involvement with specialized units (neuro and cardiothoracic ICU) within the Heart and Vascular/Cardiac ICU settings; ensures adherence to unit policies, standards of professional behavior, and supports organizational values while facilitating discharge planning and post-acute care planning.
Required Qualifications
- Graduation from an accredited school of nursing
- Licensed and currently registered nurse in the State of Connecticut
- BSN preferred not stated as mandatory but credentialing expectations include active state licensure and compliance with CNS/CTICU scope of practice
Desired Qualifications
- BSN preferred
- Clinical specialty certification highly encouraged
