Job Summary
Coordinate transitional care for patients in a cardiac/neuro-focused nursing environment. Provides supervision of non-RN personnel, assumes charge nursing duties, develops and documents individualized plans of care with patient/family involvement, and collaborates with the healthcare team to implement, evaluate, and revise care plans to meet discharge readiness. Demonstrates competency in neuro assessment and airway management for neuro patients, including suctioning and use of ventilation support as applicable. Responsibilities include supervising unit activities, ensuring adherence to unit policies and procedures, delivering education, and meeting performance expectations related to customer service, teamwork, and resource utilization. The role may include responsibilities across multiple nursing units depending on staff distribution, with location specified as West Haven, CT. Licensure in CT and RN credentials are required.
Required Qualifications
- Graduation from an accredited school of nursing
- Current Registered Nurse license in the State of Connecticut
