Registered Nurse RN - Transitional Care Coordinator

Registered Nurse RN - Transitional Care Coordinator role focused on assessing, planning, implementing, evaluating, and supervising patient care on a nursing unit. Delegates and supervises unit activities as charge nurse, coordinates care plans across the healthcare team, and provides education. Demonstrates competency in neuro assessment and care, airway management, ventilator support, and handling neuro procedures (including halo brace) within specialized units such as HVC CTICU, Cardiothoracic ICU, and CV ICU/Stepdown. Requires licensure as an RN in Connecticut and involves supervision of non-RN personnel, adherence to unit policies, and performance in line with patient care outcomes and discharge readiness.
