Registered Nurse (RN)

Provide care and services to patients residing in private homes, skilled nursing facilities, and assisted living facilities as a contributing member of the interdisciplinary team. Conduct psych-social assessments, interventions, and coordinate community resources while managing visits, documentation, and urgent needs through prioritization and triaging. Communicate independently via phone, email, and written channels with healthcare professionals, social workers, chaplains, and CNA leadership. This role operates within Case Management, Admissions, or Resource functions, requiring reliable transportation to travel across the service area. Suncrest Hospice aims to change hospice care expectations by increasing clinical visits and lowering clinician caseloads through optimized staff-to-patient ratios.
