Job Summary
Provide care and services to patients residing in private homes, Skilled Nursing Facilities, and assisted living facilities as a contributing member of the interdisciplinary team. Conduct psych-social assessments, interventions, and coordinate with nurses, social workers, chaplains, volunteers, and CNA leadership. Manage visits, documentation, and meetings while prioritizing urgent needs through phone, email, and written communication. Work independently to travel across the service area using a valid driver's license and reliable transportation. Emphasis placed on Spanish-speaking candidates.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Must be willing to drive with reliable transportation, valid driver's license, and auto insurance, to be able to travel across the service area
