Registered Nurse (RN) – Start of Care (SOC) and Visits Home Health - Bradenton, FL

Perform Start of Care (SOC) admissions and comprehensive patient assessments, then complete OASIS documentation accurately and timely. Conduct recertification, resumption of care, and routine skilled nursing visits while developing individualized care plans. Provide patient and caregiver education and coordinate care with physicians, therapists, case managers, and interdisciplinary teams. Maintain compliance with Medicare, state, and company guidelines and participate in on-call rotation as needed. Requires active Florida RN license, minimum one year of experience, and OASIS documentation proficiency. Based in Bradenton, FL, with weekly pay and flexible scheduling.
