Job Summary
Conduct Start of Care assessments, skilled nursing visits, recertifications, and discharge visits throughout Titusville and Brevard County. Develop individualized plans of care, monitor patient conditions, and provide wound care, medication management, and disease-management support. Coordinate care with physicians, therapists, and families while completing accurate OASIS documentation and maintaining compliance with state regulations. Requires active Florida or compact RN license, CPR certification, reliable transportation, and ability to work independently in the field. Weekly pay and flexible per diem opportunities available.
Required Qualifications
- Active Registered Nurse license in Florida or an active compact nursing license
- Current CPR certification
- Previous home health experience preferred
- Experience completing Start of Care visits and OASIS documentation strongly preferred
- Strong clinical assessment and documentation skills
- Ability to work independently in the field
- Reliable transportation and willingness to travel throughout Titusville and surrounding areas
- Ability to communicate professionally with patients, families, physicians, and clinical staff
- Current driver's license and auto insurance
