Job Summary
Perform initial and ongoing patient assessments and assist in developing the Hospice Plan of Care. Educate patients and families on medication administration, dressing changes, oxygen use, and hospice philosophy while acting as the primary liaison between the physician, patient, and team. Maintain accurate medical records and conduct routine supervisory visits. Participate in the agency on-call schedule and ensure compliance with all policies, laws, and regulations. Requires a Louisiana RN license, two years of direct patient care experience, current CPR certification, EMR knowledge, and a valid driver's license. Full-time benefits include an ESOP, competitive pay, and mileage reimbursement.
Required Qualifications
- Registered Nurse (RN) license required in State of Louisiana
- Minimum of two (2) years experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care
- Two (2) of experience in hospice care as an LPN may be substituted for the required 2 years of experience as an RN
- Current CPR certification required
- Current automobile insurance and valid driver's license
- Knowledge of EMR is required

- Previous Hospice experience preferred
