Job Summary
Conduct initial and ongoing patient assessments and assist in developing the Hospice Plan of Care. Educate patients and families on medication administration, dressing changes, oxygen use, and hospice philosophy. Act as the primary liaison between physicians, patients, and the hospice team while maintaining accurate medical records. Perform routine supervisory visits and participate in the agency on-call schedule. Requires a Louisiana RN license, two years of experience in oncology, acute care, or hospice, current CPR certification, EMR knowledge, and a valid driver's license. Full-time role with ESOP, competitive pay, and comprehensive benefits.
Required Qualifications
- Registered Nurse (RN) license required in State of Louisiana
- Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care
- Current CPR certification required
- Current automobile insurance and valid driver's license
- Knowledge of EMR is required
Desired Qualifications
- Previous Hospice experience preferred
- Compassionate RN with great time management skills
