Job Summary
Conduct client assessments to develop individual care plans and provide primary care in collaboration with healthcare professionals. Manage complex wounds, IV therapy, CVAD, dialysis, and medication while delivering chronic disease education and palliative support. Perform phlebotomy, specimen collection, and rehabilitative programs for medically fragile, technologically dependent, or chronically ill clients. Work full scope of practice in homes, clinics, or virtually, requiring a personal vehicle and valid driver's license. No on-call rotation; bi-weekly 45-hour schedule across days, evenings, nights, and weekends.
Required Qualifications
- Must be licensed as a Registered Nurse in the province of practice
- Must enjoy driving
- Must have a personal vehicle
- Must possess both a current driver's license and proof of vehicle insurance
- A satisfactory Criminal Records Check is required
- A Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
- Must be able to wear Personal Protective Equipment (PPE)
- This role requires a detail-oriented approach in a dynamic environment
- This role requires physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics
- Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day
- Some tasks require fine hand movements
