Registered Nurse (RN) - Rehabilitation

Coordinate the Patient Care Team's delivery of personalized, efficient care while providing patient assessment, discharge planning, and updates to the individualized plan of care. Collaborate with physicians and therapy disciplines to implement interventions, report status changes, and anticipate adverse outcomes to promote patient safety. Adhere to medication administration standards, maintain electronic medical record proficiency, and participate in process improvement initiatives using Lean Six Sigma methodology. Complete annual competencies, education, and required certifications while maintaining confidentiality and accountability for staffing within the flexible plan.
