Job Summary
Registered Nurse (RN) Rehab Nurse for full-time 12-hour night shift. Responsibilities include accurately assessing residents’ health status on admission and ongoing, documenting and updating care plans with the IDT, communicating changes to residents and families, administering medications and IV treatments, and utilizing SBARR for physician communication. Develops dynamic care plans based on residents’ needs, reviews and revises plans as conditions change, and ensures interventions are carried out safely and timely. Maintains thorough documentation of interventions and uses QAPI processes to address problems. Requires critical thinking and strong communication skills to collaborate with physicians, residents, families, and staff; ability to stand/walk for long periods and perform necessary patient documentation. Education from an accredited nursing program resulting in RN status and an active RN license in the working state; CPR/AED certification required or to be obtained within six months. Pay range: $29.39 - $48.20 per hour; Location: Clayton, NY (on-site).
Required Qualifications
- Current RN license in the state you will be working
