Job Summary
Conduct initial and ongoing patient assessments, analyze data, and create plans of care for rehab med-surg patients. Administer medications safely per state practice and organization policies while coordinating care with interdisciplinary team members. Serve as the primary point of contact for patient care coordination across hospital departments and lead health teaching activities. Act as a patient safety advocate by participating in ongoing quality improvement initiatives and evaluating treatment effectiveness. Deliver compassionate, relationship-based care utilizing evidence-based practices and professional judgment to optimize patient outcomes.
Required Qualifications
- BLS Basic Life Support
- Valid nursing license from the state of practice
- Associate of Nursing, nursing
- Diploma in Nursing
- Bachelor's degree in nursing
