Perform nursing assessments, administer medications per physician orders, and develop effective plans of care for patients with complex diagnoses. Collaborate with the nursing team, other disciplines, patients, and families to establish realistic short- and long-term goals and minimize rehospitalizations. Delegate patient care responsibilities to staff and supervise implementation of interventions while communicating patient information between shifts.
