Job Summary
Conduct client visits to support treatment plan goals, including healthy eating, medication education, and exercise coordination. Assist with applying for medication benefits, coordinate with medical providers, and accompany clients to appointments. Administer IM injections and monitor vitals monthly while ensuring medication orders are sent to the pharmacy and delivered as indicated. Work within the Assertive Community Treatment Team in both office and community settings, collaborating with psychiatrists and other specialists. Maintain a Monday-Friday schedule with an on-call component in Dover, DE.
Required Qualifications
- Associate's degree in Nursing
- Minimum 1 year of related experience
- Valid RN license in the State of Delaware
- Valid Driver's License and reliable transportation
- Must pass criminal background check and drug screen
- Relocate before starting work
