Job Summary
Provide nursing care in a post-acute setting under RN delegation, including administration of medications and IV therapy within LPN scope, documentation in the EMR, and development and execution of individualized care plans. Collaborate with the interdisciplinary team to assess patient needs, communicate changes in condition, and contribute to discharge planning. Deliver health teaching to patients and families, ensure adherence to infection control and HIPAA policies, and participate in shift handoffs and team conferences to optimize patient outcomes.
Required Qualifications
- High School Diploma or Equivalent
- Graduate of an accredited LPN program
- Valid State Appropriate LPN license
- Current CPR certification
