Assess patient condition during admission and each shift, identifying and reporting status changes. Perform procedures, monitor patients, and document care administration in medical records. Respond promptly to patient requests while anticipating needs to resolve issues proactively. Collaborate with medical providers and the multidisciplinary team to coordinate, evaluate, and deliver a holistic treatment plan. Serve as the interface between healthcare providers and community support systems to manage continuation of care for specialized patient groups.
