Job Summary
Utilizes the nursing process to identify and achieve patient goals through assessment, planning, implementation, and evaluation. Assesses physical, psychosocial, and environmental factors to develop a comprehensive nursing care plan tailored to achieve desired health outcomes. Collaborates with an interdisciplinary team to ensure optimal patient health and well-being. Teaches patients and caregivers about health care and disease management according to their needs and levels of understanding, while documenting ongoing requirements and comprehension.
Required Qualifications
- Unencumbered current Registered Nurse license in the state where the HomeCare agency is located or compact state, if applicable.
- Minimum one (1) year experience as a RN / professional nurse.
Desired Qualifications
- Experience in home care is desirable.
