Job Summary
Provide skilled nursing care in a home health setting for medically fragile children and young adults. Deliver one-on-one, in-home care, administer prescribed medications, treatments, and therapies, and perform ongoing patient assessments and clinical evaluations. Educate patients and families to support improved outcomes and coordinate care with families, providers, and care teams. Flexible scheduling with full-time, part-time, and PRN opportunities; 8-, 10-, 12-, or 16-hour shifts available. Preferred pediatric or home health experience and high-acuity skills (trach, vent, G-tube).
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license (Pennsylvania)
- License acquired 1 year prior to application
- Ability to provide care in a home setting
