Registered Nurse (RN) - Primary Care West Haven

Support providers including physicians, nurse practitioners, and physician assistants in delivery of patient care under immediate supervision. Provide phone and in-person triage to assess patient urgency, coordinate care, and determine treatment timelines. Administer injections, record vital signs, and manage medication refills and prescription orders. Complete patient forms, pull charts, and document all tasks in the EHR while maintaining a neat work area. Assist with patient follow-ups, appointment scheduling, and inquiries. Maintain flu vaccination proof and hold a current State RN license. Located primarily in Guilford with travel to West Haven, New Haven, Milford, and Wallingford.
