Job Summary
RN in Internal Medicine group provides support to physicians, nurse practitioners, and physician assistants in patient care. Responsibilities include medication refills, completion of patient forms, patient reminder calls, follow-up on medication changes, lab results, scheduling future appointments, and liaison between providers and callers. Performs triage of patients, conducts phone triage to determine urgency and need for intervention, places prescription requests with pharmacies, sets up referral contacts, pulls and prepares patient charts, answers patient inquiries, maintains tidy work area, and documents all patient care in the EHR. Requires demonstrated nursing theory/practice knowledge, proficiency with EHR/healthcare software, and strong communication skills. RN degree from an accredited program and current CT RN license required. Hours are Monday–Friday 8:30am–5:00pm; position located primarily in Guilford with travel to West Haven, New Haven, Milford, Wallingford and other locations as needed.
