Job Summary
Remote RN position focused on population health outreach. Responsibilities include non-face-to-face post-discharge outreach calls, reviewing discharge summaries and medications, conducting medication reconciliation, assessing patient understanding of discharge plans and adherence, coordinating with providers and care teams, and documenting outcomes in the EHR. Tasks also encompass scheduling primary care appointments, addressing care gaps (hypertension/diabetes), delivering hypertension-focused education, and participating in quality improvement initiatives to support CMS requirements and readmission prevention. Requires nursing education as specified and an active RN license; work is remote for TN residents with a base location in Jacksonville, FL.
Required Qualifications
- Associate degree in Nursing (ADN) or RN diploma/certificate
- Current RN license recognized in state of practice (RN compact/multistate license acceptable)
- Minimum 1 year RN experience
