Job Summary
Remote population-health RN responsible for initiating post-discharge phone outreach to discharged patients, reviewing discharge summaries and medication lists, conducting medication reconciliation, assessing understanding of discharge plans, documenting interactions in the EHR, coordinating with providers and care teams, scheduling follow-up appointments, and educating patients on hypertension management and preventive care to support readmission-prevention efforts.
Required Qualifications
- Associate degree in Nursing or RN diploma/certificate with current RN license
- One (1) year experience as a registered nurse
- Current RN compact/multistate license recognized by NCSBN or licensed to practice in state
- RN license recognized by the state of work
