Registered Nurse (RN)- Population Health, Remote, Full-Time, Days

Registered Nurse (RN) in Population Health, remote position for Tennessee residents, full-time days. Responsible for initiating post-discharge phone outreach with discharged patients, reviewing discharge plans and medication lists, conducting CMS-required follow-up calls, performing medication reconciliation, assessing patient understanding and adherence, coordinating with care teams, scheduling follow-ups, documenting in the EHR, and supporting readmission prevention. Provides hypertension-focused education, identifies escalation needs to providers, and participates in quality improvement related to department outcomes. Requires an ADN with RN licensure (compact/multistate accepted); Bachelor's degree preferred; 1 year of RN experience. Remote work is available for TN residents; position located within Prisma Health ambulatory services with related supervision by Nurse Manager.
