Job Summary
Remote Registered Nurse (RN) in Population Health. Responsible for initiating post-discharge outbound calls to discharged patients, conducting queries on patient experience and recovery, coordinating follow-ups with care teams, and providing education and outreach to patients and families. duties include medication reconciliation, assessing understanding of discharge plans, scheduling follow-ups, documenting interactions in EHR, and supporting readmission-prevention efforts. Role emphasizes hypertension-focused education, collaboration with providers and care coordinators, and adherence to CMS requirements. Education: ADN or nursing diploma with RN license; experience: ~1 year RN; work primarily remote for Tennessee residents; Day shift.
Required Qualifications
- Associate degree in Nursing (ADN) or equivalent
- Current RN license (NCSBN multistate or equivalent)
- One year of RN experience
- Ability to perform telephonic post-discharge outreach
- Knowledge of discharge planning, medication reconciliation, and CMS requirements
- Hypertension-focused education experience preferred
- Proficiency with EHR/documentation
- Ability to educate patients and coordinate with care teams
- Remote work ability for TN residents
