Job Summary
Remote position for TN Residents performing non-face-to-face post-discharge outreach, medication reconciliation, patient education, and care coordination to support CMS-readiness and readmission-prevention goals; duties include reviewing discharge plans, coordinating appointments, documenting interactions in EHR, and providing hypertension-focused education. The role requires an Associate degree in Nursing (BSN preferred) and an active RN license; the assignment includes a remote TN option and an in-person site at Blount Memorial Hospital in Pittsfield, MA.
Required Qualifications
- Associate degree in Nursing (ADN) or RN diploma/certificate with current multistate license (NCSBN Compact) or license to practice as RN in state of work
- Holds a current RN compact/multistate license recognized by NCSBN or licensed in state of work
- One year of RN experience
