Registered Nurse (RN)- Population Health, Remote, Full-Time, Days

Initiate non–face-to-face post-discharge outreach calls to discharged patients within CMS-required timeframes, review discharge plans and medication lists, conduct medication reconciliation, assess understanding of discharge instructions, provide hypertension-focused education, and coordinate follow-up with primary care teams. Responsibilities include documenting interactions in the EHR, identifying potential complications, scheduling and securing timely appointments, supporting readmission prevention, and collaborating with care teams to ensure continuity of care, all while operating as a remote RN for Tennessee residents based in a Saint Paul, MN area facility. Requires ADN with RN license and at least 1 year of RN experience; Bachelor’s degree in Nursing preferred.
