Registered Nurse (RN)- Population Health, Remote, Full-Time, Days

RN Population Health role with Prisma Health: provide remote post-discharge outreach via phone to discharged patients, review discharge plans and medication lists, perform medication reconciliation, assess understanding and adherence, document interactions in the EHR, escalate to providers as needed, and coordinate scheduling and follow-up with primary care. Focuses on CMS-required timelines, hypertension-focused education, and readmission prevention. Collaborates with care teams, provides education to patients/families about discharge instructions and when to seek urgent care, and participates in quality improvement initiatives to improve department outcomes. Requires an Associate degree in Nursing (Bachelor's preferred), current RN license, and at least 1 year of RN experience; remote position available to TN residents (Nashville, TN area).
